CONFUSED DRUG NAMES AND HIGH ALERT MEDICATIONS *

The UCSC Student Health Center Pharmacy posts and supplies the ISMP lists for Confused Drug Names (Look-Alike / Sound-Alike) Medications and the ISMP High Alert Medications. Signage is added to storage areas for situations that could be of risk in the department.

These lists are reviewed at least annually (after inventory) and updated as needed.

When new products are received, they are referenced against the ISMP Confused Drug Names and High Alert Medications List and alerts are documented in the Pharmacy software inventory - with a hard stop and override required for these medications.

Tallman letters are used whenever available and the electronic medical record pharmacy picklist is reviewed and managed to prevent inadvertent errors with similar names and dosages.
