1. Record temperatures on log sheet:

Thermometers NIST calibrated :

Refrigerator: + 2-6 C
Freezer: -20-30 C
Incubator: + 34-38 C
Room: + 18-28 C
Waterbath: + 55-58 C

2. Quality control specimens performed by Clinical Lab Scientist

Hematology:

  • Coulter 4C- ES Controls  Level 1, 2, and 3
  • Record results in Harvest LIS

Urinalysis:

  • Quantimetrix Dropper Plus Level I and II commercial controls: perform Chemstrip 10MD dipstick analysis using Urisys 1100 analyzer
  • Record results in Harvest LIS

Microbiology:

  • Antibiotic sensitivities weekly with patient specimens using E. coli and S. aureus.
  • Appropriate control organisms for positive/negative testing weekly for bacitracin (Taxo A) and novobiocin disks.
  • Appropriate control organisms for positive/negative testing daily for catalase,  indole, oxidase, and gram stain in conjunction with patient specimens
  • Record results on appropriate log sheets
  • Commercial media is inspected for Hemolysis (as appropriate), Contamination and Damages upon arrival and recorded in a log.  Samples from manufactured lots meet the specification listed on the Certificate of Analysis and/or instructions for use at www.HardyDiagnostic.com.  This is a CLIA reduced protocol for QC and has been implemented by IQCP.

Pregnancy Testing:

  • Urine: run positive and negative controls every 30 days, or a new lot number or new operator; observe internal control for each run
  • Serum: run positive and negative controls daily with patient specimens, observe internal control for each run
  • Record results on appropriate log sheet

Mononucleosis Testing:

  • Whole blood: run positive and negative controls when using new kit, observe internal control for each run
  • Record results Harvest LIS

Rapid Strep Testing:

  • Positive and negative controls on each new kit, observe internal control for each run
  • Record results Harvest LIS

Occult Blood Testing:

  • Positive and negative controls run daily with each patient (controls located on each test card).
  • Record results Harvest LIS.

Whole Blood Glucose Testing:

  • Run Low and High control each day of testing.
  • Record results Harvest LIS.

Rapid Flu Testing:

  • Positive and negative controls on each new kit
  • Record results Harvest LIS.

D-Dimer Testing:

  • Positive and negative controls every 30 days. Run daily control with each day of patient or QC testing.
  • Record results in Harvest LIS.

Chemistry:

  • BRT Controls 1 and 2 monthly and/or new lot of reagent and/or new shipment.
  • Record control results in Harvest LIS and put print out in the Piccolo binder.
  • Run Daily iQC with each day of patient testing.  Put printout in Piccolo binder.

Expiration dates are checked continuously and expected expirations are noted on the Laboratory calendar to alert staff.  General expiration date checking throughout the Laboratory is done monthly and recorded on an appropriate log.
