LABORATORY CONTINGENCY PLAN AND EMERGENCY REFERENCE LABORATORY TESTING

Policy: 

A.) Clinical Lab Scientist(s) & Lab Assistant Available

  • Regular laboratory operation and services
  • Same day clinic RN's may be available to draw blood in Same Day Clinic or assist in lab

B.) Clinical Lab Scientist(s) Aailable, No Lab Assistant

  • Regular laboratory operation and services
  • Call limited appointment staff if necessary
  • Same Day Clinic RN's may be available to draw blood in the Same Day Clinic or assist in the lab
  • Utilize medical assistant or support staff from other work areas
  • There may be longer waiting times for phlebotomy and test results
  • Routine phlebotomy (i.e. STI testing) could be deferred

C.) Lab Assistant Available, No Clinical Lab Scientists

Note: Lab assistant unable to perform any testing without direct CLS supervision

  • Call limited appointment CLS staff
  • Same Day Clinic RN's may be available to draw blood in the Same Day Clinic or assist in the lab
  • Utilize medical assistant or support staff from other work areas
  • Inform clinical staff of availability of laboratory testing
  • Phlebotomy for send out may be done; see section E. for emergency or stat reference lab testing
  • All other routine in-house testing (i.e. CBCs, mono testing, urinalysis) may be sent out to Quest laboratory
  • Urine pregnancy testing may be done in the Same Day Clinic by trained personnel (controls are done once per kit lot number)
  • Throat cultures: set up for in-house culture if CLS is returning the next morning; otherwise, send out to Quest Laboratory (Amies culturette)
  • Urine cultures: set up for in-house culture if CLS is returning the next morning; otherwise, send out to Quest Laboratory (clean-catch urine specimen)
  • There may be longer waiting times for phlebotomy and test results
  • Routine phlebotomy such as STI testing could be deferred
  • Emergency or stat laboratory testing may be done at Quest Laboratory (see section E.)

D.) No Staff Available In Laboratory

  • Call limited appointment staff
  • Utilize medical assistant or support staff from other work areas
  • Phlebotomy for sendouts may be done by Same Day Clinic RN's. See section E. for emergency or stat reference laboratory testing
  • All other regular in-house testing (i.e. CBCs, mono testing, urinalysis) may be sent out to Quest Laboratory
  • Routine phlebotomy and testing could be deferred until laboratory staff returns
  • Urine dipsticks may be done in the Same Day Clinic by trained personnel (after controls are done)
  • Urine pregnancy testing may be done in the Same Day Clinic by trained personnel (controls are done once per kit lot number)
  • Throat and urine cultures may be set up by trained personnel only if the Clinical Lab Scientist is returning the next morning; otherwise, they should be sent out to Quest Laboratory

E.) Stat Or Emergency Sendouts To Quest Laboratory In San Jose

  • For stat laboratory tests which are not performed in-house (or if CLS is not available) and need to be send to Quest Lab, call: 1-800-288-8008, ext. 3 and request a stat courier. It usually takes a courier up to 3 hours to arrive here. Inform Quest of where the specimen(s) may be located
  • During the regular workday hours, process specimen and keep int he laboratory for pickup. If it is close to the end of the work day, place in lockbox at the back door.
  • Label the Quest Lab request slip as "STAT" on the top portion in red. Place specimen(s) in the specimen bag and also mark "STAT" on the plastic specimen bag
  • Complete the "CC:" section on the left side of the request slip  with the clinician's full name, address, and the fax number. Also indicate if the results need to be called to a clinician's home number after clinical hours.
